Eldercare is difficult. Getting started with Eldercare is easy. Call today to take advantage of your benefit or to get more information.
Fact Finding Consultation
The Fact Finding Consultation connects you - and any family members you’d like to include - with your Health Care Manager to discuss the caregiving situation. You can share your concerns, ask questions, and pinpoint the issues that you’d like to address. After the initial call, your Health Care Manager will develop a report to discuss with you in a follow-up call. The report prioritizes the existing issues, identifies any additional areas of concern, and presents recommendations to help your family manage them. You and your Health Care Manager will determine the next right steps.
In-home Assessment
If you’d like, a registered nurse will visit your parent or loved one at home anywhere in the U.S. to perform a holistic assessment. The nurse will evaluate your loved one’s safety; physical, social, mental, emotional, spiritual, and physical well-being; unmet needs; and cognition. Based on the needs identified in the assessment, your Health Care Manager will develop an action plan for your family. Your ongoing relationship allows you to discuss the plan with your Health Care Manager and receive coaching on how to implement it.
